✅ Bloodworth: To apply to purchase controlled substances from Bloodworth, you must provide the following:
- Pharmacy photos (1. pharmacy area 2. store sign 3. retail area)
- There is an additional 90-day waiting period to apply for C2s
✅ Capital Drug: To apply to purchase C3 - 5s from Capital Drug, you must complete their controlled substance application (request a copy of the application by emailing sales@pharmacymarketplace.com).
✅ Prescription Supply: A store visit is required to apply to purchase controlled substances from Prescription Supply. Please contact sales@pharmacymarketplace.com for more information.
✅ TopRx: To apply to purchase C3 - 5s from TopRx, you must provide the following:
1. Complete TopRx's controlled substance application (request a copy of the application by emailing sales@pharmacymarketplace.com)
2. Provide a 90-day Summary Usage Report for both Controls and Non-Controls (Excel-format Preferred) containing the following:
- Pharmacy Name and Address
- Date Range
- Drug Name/Strength
- NDC
- Number of Prescriptions
- Quantity Dispensed
Email completed TopRx controlled substance applications and 90-day usage reports to sales@pharmacymarketplace.com.
❌ Auburn: Customers can apply to purchase C3 - 5s directly from Auburn by emailing PMPSales@auburnpharm.com
❌ BluPax: BluPax will offer controlled substances beginning in 2025. More details to come.
❌ Independent Pharmacy Cooperative (IPC): IPC does not currently have a DEA license.
❌ Second Source Rx (2SRx): 2SRx does not currently have a DEA license.
