  • NDC:

  • Quantity:

  • Lot #:

  • Expiration Date:

  • Reason for Return:

Please Note:

All of our secondary wholesale partners will no longer accept returns on bottles that have any stickers adhered to the manufacturer’s label. As part of DSCSA requirements, they must scan the 2D barcode on the original label, and if a price sticker removes or damages that information, the product cannot be resold.

Tip: Place any price stickers on the bottom of the bottle to avoid damaging the manufacturer’s label.
